We understand that there may be instances where you are not comfortable reporting serious issues directly to anyone within the organization. This form serves as a last resort to prompt an investigation or alert the company to significant concerns. This form should only be used by current or recently separated employees to report issues of substance that occurred at work. This form should not be used to present any social media posts, extracurricular activities, or relationship statuses
Reporting anonymously means you disclose that you believe you have experienced (or are aware of an incident involving) a serious situation that you wish to make the company aware of. Because the reporting is anonymous, you may be contacted regarding a formal investigation and your statements during that investigation will be part of the official record. Please note, however, that the company may be limited in its ability to investigate or respond to the incident if it does not have sufficient information from which to follow up on the report.
Anonymous reporting may be appropriate for a variety of circumstances including but not limited to:
Harassment or Discrimination: If you have experienced or witnessed harassment, discrimination, or any form of unfair treatment based on race, gender, sexual orientation, religion, or other protected characteristics.
Workplace Violence: If you are aware of threats or acts of violence, including physical aggression, intimidation, or bullying in the workplace.
Safety Violations: If you observe unsafe working conditions or practices that could potentially harm employees or violate safety regulations.
Ethical Violations: If you have knowledge of unethical behavior, such as conflicts of interest, fraud, theft, or other forms of misconduct that compromise the integrity of the organization.
Compliance Issues: If you are aware of actions that violate company policies, legal regulations, or industry standards, including financial misconduct or regulatory non-compliance.
Retaliation: If you fear retaliation for reporting a concern directly to management or HR and believe that your anonymity is essential for protection.

Anonymous reporting has limitations on what can be effectively investigated. Without verifiable details, it is challenging to corroborate claims, which may restrict the scope of the investigation or the ability to launch an investigation. Depending on your position within the organization, you may or may not be made aware if a formal investigation is conducted and you may or may not be informed of any resulting disciplinary actions resulting from the investigation. Investigation Limitations: Anonymous reports can initiate an investigation, but the lack of identifiable information may limit the ability to substantiate claims. A formal investigation will place the claims on record, allowing for a more thorough examination.
Use Verifiable Details: Whenever possible, please provide specific, verifiable details about the issue you are reporting. Avoid generalities or emotionally charged statements, as these can hinder the investigation process.
Duplicate Submissions: Refrain from submitting duplicate reports. Allow the organization sufficient time to investigate and address the concerns raised. Duplicate submissions can complicate and delay the investigation process.
IP Address Disclosure: To maintain the integrity of the anonymous reporting system, we do not provide clients with IP addresses if they are captured when you submit this form. IP addresses may be reviewed by Peopleish site administrators for patterns of submission to eliminate duplicate reports and ensure the reliability of the information provided. This is to ensure the integrity of the information presented here. Rest assured, every effort will be made to protect your identity. This tool should not be used to defame, discriminate against, bully or harass any employee. Discrimination or harassment using this platform waive any claims to anonymity. Any incidents of threats, violence, or other similar conduct while using this platform will be taken seriously and immediately sent to the appropriate law enforcement.
